A newly remodeled transitional care unit offers a customized experience focused on rehabilitation and service, while a continuum of care ranging from assisted living to transitional and long-term care fosters peace of mind. Spiritual care also plays an integral role in our community, providing support to residents and families from all faiths and traditions.

Augustana Kenwood Place Studios
Linger over a hearty breakfast, join a card game or lively discussion and celebrate a special birthday with family and friends. Spacious studio apartments are completely wheel-chair accessible at Augustana Kenwood Place, offering convenience and country charm. You’ll also find individualized health care plans, an on-site nurse, personal call system, weekly housekeeping and a full calendar of events and activities.
For those who need extra assistance performing daily activities but don’t require skilled nursing care, home health aides are available. With an array of services tailored to your unique needs and preferences, you’ll have more opportunities to concentrate on the things you enjoy most.
Augustana Mercy Health Care Center
Augustana Mercy Health Care Center offers a broad continuum of care, including transitional and long-term care. The transitional care unit provides a bridge between hospital and home where individuals recovering from illness or injury can have the time they need to heal.
Long-term care serves those whose medical needs require specialized assistance from licensed nurses. Physical and occupational therapists, dieticians and other professionals also help sustain physical and emotional well being.
Programs for stroke patients, customized therapeutic services and strong connections with specialists are all part of the experience we offer, and being attached to Mercy Hospital means no ambulance rides are needed. Special care for people with dementia is also available and includes a secure patio and inviting gardens with experienced, caring professionals close at hand.
More than 75 active volunteers bring companionship and camaraderie to residents throughout the year. In addition, churches take turns providing non-denominational services and chaplains are readily available for consultations and spiritual support.
For those needing to regain skills after illness or injury, transitional care can help quicken recovery. Augustana Mercy’s transitional care unit offers a newly enlarged and expanded therapy center, nearby dining room, and private room suites with phones and televisions. Residents may eat in their rooms or join health care residents for meals and other activities in a communal dining room.
Care is provided by compassionate professionals who share your goals to make a smooth transition between the time you spend in a hospital and the time you are ready to go home. The focus is on you and your needs and includes consultations to devise custom rehabilitation routines that suit your lifestyle. Staff members have received specialized training on hip, knees and wound care and have access to clinical education and training through Mercy Hospital.
